Part Two: Program Application

1. Insurance Requirement *

Sampling Providers must demonstrate to RESNET a minimum insurance coverage of $1,000,000 in general liability coverage and $1,000,000 in professional liability coverage [RESNET®-HERS® Std. section 609].

Please attach a copy of your organization's certificate(s) of insurance verifying coverage for the period covered by this application year.

 

2. Minimum Standards for a Sampling Provider *
Please attach a complete copy of your Sampling Provider's Operation Policies and Procedures Manual.

Please explain how your program meets each of the following requirements of the RESNET-HERS Standard. Each item below must be addressed separately and cannot just reference your Policies and Procedures Manual.

a. Eligibility & Records Maintenance [601 & 602] - How will your organization ensure proper documentation and oversight of the sampling process? Describe how you will verify that sampling is applied appropriately, that required records are maintained, and that inspection data is complete and accurate across projects.*

 

b. Sampling Plan [603] - How will your organization oversee the development and maintenance of Sampling Plans? Explain how you will ensure plans are properly created, updated when project conditions change, and submitted to RESNET as required. Your response should reflect how you will review and document changes such as adjustments to Threshold Specifications, additions of Sampled Features, and the removal of units from sampling to be labeled as Confirmed Ratings.*

 

c. Energy Modeling [604] - How will your organization verify that dwelling unit modeling and worst-case analysis meet program standards? Briefly describe your QA procedures for how you will review modeling groups, confirm worst-case model accuracy, and ensure proper documentation and Registry submissions.*

 

d. Labeling [605] - How will your organization ensure required sampling labels are included in project reports? Briefly describe how you will verify that unit and/or building labels meet RESNET's sampling protocol requirements.*

 

e. Procedures for Inspections & Testing [606] - How will your organization oversee the creation of Sample Sets and ensure Qualification Sets are conducted before sampling begins? Describe how you will verify Sample Set composition is compliant, and that Qualification Sets meet the minimum requirements before sampling of each feature begins.*

 

f. Sample Verification Process [607] - How will your organization ensure Sampling Verifications meet minimum quantity and representative selection requirements? selection requirements? Describe how you will confirm that at least 20% of each Sampled Feature will be verified within each Sample Set, that selections will be representative of the project scope, and that failures will be addressed according to RESNET procedures.*

 

g. Procedure for Resolving Failures [608] - How will your organization oversee the handling of failures and requalification for Sampled Features? Describe how you will ensure sampling stops when a failure occurs, that corrective actions and requalification will be properly completed, and that all required failure documentation will be maintained.*

 

 

3. Quality Assurance Process - How will your organization oversee both file and field quality assurance for sampled ratings? Describe how you will track and comply with the file QA requirements outlined in section 904.3.2.7, and how you will manage the field QA parameters in section 904.3.3.1.6. Your response should also address how you will meet the additional field QA requirements for multifamily projects in section 904.3.4.*
